Transition Care Community
Transition Care Community is short-term program for older people who require support at home after a period in hospital.
Transition Care allows up to 12 weeks to provide goal-orientated, low intensity therapy, and assists you as you rehabilitate back into your own home.
Transition Care Community at Amana Living can provide the following services:

- Domestic Assistance

- Meal assistance / preparation
- Personal Care
We also offer physiotherapy, occupational therapy, social work and clinical support on site at Mosman Park.
Fees for Transition Care
A client in Transition care community will pay the basic daily care fee, which contributes to the cost of their care. This is 17.5% of the single aged pension and applies to both single and married transition care clients.

Amana Living’s transition care places are currently spread across two service locations at Mosman Park and Bull Creek. Transition Care Bull Creek opened in January 2016 as part of the new dementia services centre.

The Transition Care Program (TCP) is a Commonwealth initiative that provides short-term restorative care for older people after a hospital stay. The aim is to help them regain independence and give them more time to make a decision about their longer-term living options. Services include nursing and personal care, low intensity physiotherapy and occupational therapy, as well as social work support, nursing and personal care.
There is a daily care fee set by the Government at 85% of the basic daily rate of the single pension. For Transition Care Community the rate is 17.5% of the basic daily rate of the single pension.
When an older person is in hospital for an extended period, the hospital treating team may recommend transition care for them before returning home or moving into residential care. The hospital team will arrange an assessment by an Aged Care Assessment Team and coordinate a referral to the transition care service. Our liaison nurse then visits the patient in hospital to confirm that our service is suited to their needs, and assist with the transfer.
Amana Living offers a mix of dementia-specific, older adult mental health (OAMH), and frail-aged transition care places. Our sites have experienced clinical teams, including dementia and OAMH specialists, and excellent facilities. These include ensuite accommodation, well-equipped therapy rooms and large lounge/dining areas.
