From hospital to an Amana Living Home
The Transition Care Program (TCP) is a Commonwealth-State initiative that provides short-term restorative care for older people after a hospital stay.

The aim is to help them regain independence and give them more time to make a decision about their longer-term living options.
The average length of stay in transition care is seven weeks, but clients may stay for up to 12 weeks if they need extra time. Services include nursing and personal care, low intensity physiotherapy and occupational therapy, as well as social work support, nursing and personal care.
The program is led by GPs with geriatrician support. Each week, they meet with a multi-disciplinary team, including allied health practitioners and nursing staff, to discuss clients’ progress toward optimal health and independence.
