ACCESSING HOME CARE

Help your patient gain access to Home Care in 7 steps

If you or your patients find yourself stuck, call the team at Amana Living on 1300 26 26 26. Leonie and Andrea have helped hundreds of people get their entitlements. It’s an obligation-free service.

Professionally trained. Naturally kind. 

1The patient visits their GP to ask for an ACAT referral.

You, the GP, refer them for an ACAT assessment via www.myagedcare.gov.au/health-professionals

OR

Ask your patient to call us and we will guide them through the process.

2The ACAT assesses your patient in their home

Your patient will be contacted by the ACAT to arrange to visit them and work out the services they may need.

3The patient completes an Income Assessment with Services Australia

The patient can call us on 1300 26 26 26 (option 2) and we can mail the form to them (if applicable).

4Your patient will receive three letters

1. ACAT letter approving their care level.
2. Services Australia letter with their income assessment (if applicable).
3. Home Care Package Assignment letter - this may take 3 to 6 months.

5The patient receives the assignment letter

They have 56 days before the offer expires to call us, organise an appointment and activate the funds.

6Compare service providers

With their letters in hand – they call their preferred providers to compare services and costs, and hopefully we’re one of them.

7Select a service provider and sign up

Their services can start immediately.

At Amana Living, we offer a wide variety of home care support and services so you can enjoy an independent life.  Whatever your patient's needs, we’ll work with you to put a plan in action. If your patient is happy to talk to us directly, we offer an obligation free service where we aim to find the best services possible for their needs. 

Wherever they live in the Perth metro area or Kalgoorlie, and whatever level of care you require, we can provide extra care at home so you can keep living independently.

    What to consider when choosing a home care provider

    We understand that choosing a home care provider can be confusing. Some of the things to consider are:

     

    Not for profit vs private company

    Amana Living is a not for profit, so the focus is on you, our client, instead of making profit for shareholders.

    Expertise

    Amana Living is one of WA’s largest providers of care and services for older people. We have been serving the WA community since 1962, so you can be assured of the high quality of our care.

    Values and compassion

    As part of the Anglican community, Amana Living has high integrity and adheres to values of Compassion, Justice, Hope and Inclusiveness, no matter what your religion in the delivery of home care services.

    Scope of services

    Amana Living not only provides home care, but also social day clubs for seniors, day clubs for clients with dementia, dementia support services, retirement villages and nursing homes.

    Costs

    All home care providers are required to publish their prices in a standard format to make it easier for you to compare. The My Aged Care website lists the costs of all providers available, or call us and we’ll walk you through it.

Both my parents really like her and she reciprocates in the way that goes above and beyond her duties.
