Mosman Park

At Transition Care Mosman Park there are 46 frail-aged places (not dementia-specific).

The custom-designed facility is a redevelopment of the original residential care centre, and is co-located with Dorothy Genders Village in an attractive, quiet location between river and sea.

Clients have single or shared rooms with ensuites, and access to communal lounge/dining area.

The centre also provides a mini-apartment, giving clients the chance to test their capacity to live independently before moving out of transition care.

When an older person is in hospital for an extended period, the hospital treating team may recommend transition care for them before returning home or moving into residential care.

The hospital team will arrange an assessment by an Aged Care Assessment Team and coordinate a referral to the transition care service.  

Transition Care Mosman Park is dedicate to the frail-aged. If the team recommends that the patient be transferred to a centre that offers frail-aged places, they may contact Amana Living.

Our liaison nurse then works with the hospital transition care coordinators to ensure a smooth move from hospital to Amana Living. The nurse will visit the patient in hospital, confirm that our service is suited to their needs, and assist with the transfer.

What is transition care?

The Transition Care Program (TCP) is a Commonwealth initiative that provides short-term restorative care for older people after a hospital stay. The aim is to help them regain independence and give them more time to make a decision about their longer-term living options. Services include nursing and personal care, low intensity physiotherapy and occupational therapy, as well as social work support, nursing and personal care.

How much does transition care cost?

There is a daily care fee set by the Government at 85% of the basic daily rate of the single pension. 

How do I find out if I’m eligible for transition care?

Amana Living offers a mix of dementia-specific, older adult mental health (OAMH), and frail-aged transition care places. Our sites have experienced clinical teams, including dementia and OAMH specialists, and excellent facilities. These include ensuite accommodation, well-equipped therapy rooms and large lounge/dining areas. Where are Amana Living’s transition care centres located?

Amana Living’s transition care places are currently spread across two service locations at Mosman Park and Bull Creek. Transition Care Bull Creek opened in January 2016 as part of the new dementia services centre. Our Kinross dementia ‘hub’, currently under development, will also offer transition care.
