Recently launched the McCusker Nurse program is an innovative free service that offers tailored one on one support, information and guidance to people living with dementia, their carer/s and family members.
The McCusker Nurse
- is a specialist Registered Nurse with experience in dementia care who will offer guidance, advice, support and reassurance;
- will provide information about the resources available for living the second half of life with dementia from the time of diagnosis and support people to maximise their quality of life; and
- will guide you to exisiting dementia services so that you can make the best choice.
Who can access the service
People with dementia and their carers living in the North Metropolitan Region of Perth, including the cities of Stirling, Joondalup and Wanneroo. A detailed list of the suburbs where the service is currently available can be found on the Suburb List (pdf).
Referrals to the service can be made by
- the person living with dementia;
- carer;
- family member;
- medical specialist: GP, Geriaticians, Physco geriatrcians;
- Allied Health: Aged Care Assessment Team (ACAT), social workers, physiotherapists and occupational therapists, community nurse groups, Incontinence Advisory Service, diabetic educators, Parkison and Neuro nurses; and
- service providers: HACC, EACH, EACHD, Extra Services.
