Extended Aged Care at Home (EACHD) packages are individually tailored programs of care to help older people with dementia that experience difficulties with daily life because of their dementia.
Both the person with dementia and family carers are involved in developing the package of care that will best meet the person’s needs.
EACHD packages are very flexible and can include nursing care, personal care, social support, home help, respite, links with behaviour management specialists, podiatrists, physiotherapists and other health providers as required.
Who is eligible for EACHD?
Each EACHD program is intended for frail older people who:
- Are unable to live independently in the community because the symptoms of their dementia significantly impact on their lives
- Have been assessed by an Aged Care Assessment Team (ACAT) as needing a high level of care
- Are eligible to receive high level residential care, but
- Have expressed a preference to live at home, and
- Are able to do so with the assistance of an EACHD package.
A well qualified and experienced nurse provides on-going clinical support, especially when the person with dementia has complex technical care needs.
