Extended Aged Care at Home (EACH) packages are intended for frail older people with complex care needs who wish to remain living in their own homes.
EACH packages are very flexible and can include nursing care, social support, home help, respite, links with behaviour management specialists, podiatrists, physiotherapists and other health providers as required.
Who is eligible for EACH?
Every EACH program is intended for frail older people who:
- Are unable to live independently in the community
- Have been assessed by an Aged Care Assessment Team (ACAT) as needing a high level of care
- Are eligible to receive high level residential, but
- Have expressed a preference to live at home, and
- Are able to do so with the assistance of an EACH package.
A well qualified and experienced nurse provides ongoing clinical support.
