Tailored to meet the individual requirements of older people who wish to remain in their own homes. Community Aged Care Packages offers a range of services including:
- Personal care
- Shopping
- Cleaning and laundry
- Meal preparation
- Transport
- Gardening
To access this service you will need a referral to an Aged Care Assessment Team (ACAT) from your local General Practitioner. A member from an ACAT will determine your eligibility for a Community Aged Care Package.
This service operates in the Perth metropolitan area and in the City of Kalgoorlie – Boulder.
Charges for these services are determined in accordance with the Commonwealth Department of Health and Ageing Guidelines.
